Dr. Schlesinger talks with a patient about his refined and artful technique for belly buttons after a tummy tuck.
The word for a belly button is umbilicus and to umbilicate means to make something go inward like this so there’s a whole trick to doing a belly button. You’ve got to know where it is and I make my belly buttons 15 mm long by 8 mm wide. And we take all the fat around it. So if you think about it, since the skin on the belly button kind of goes in, like this, you can’t have a lot of fat on this side and a lot of fat on this side because it just won’t work. So you de-fat on the inside around where the belly button’s going to be. You sew the belly button down to the abdominal wall. And the belly button has two blood supplies: one from your skin because it’s part of your skin and one from your liver because that’s how your mother fed you when you were inside of her. The umbilical cord comes from the placenta with nutrients into the liver. So there’s one artery and two veins from the liver to the belly button. So when you cut around it from the skin, it’s still alive. Then you anchor it down to the abdominal wall and then you de-fat the skin of the abdomen. Then as you sew it down, it umbilicates, it turns inward. The next part of this is the suture line, the junction of the belly button and the abdominal skin. No stitch can cross that line because if a stitch crosses that line it leaves a scar. So we sew underneath and then we put superglue around that as well. That’s a very special technique for doing belly buttons. I looked at all the techniques in books and I didn’t like any of them, my belly buttons come out really well and I’m very proud of them.
